Review a Note
Quick access: Note review
Overview
Review checks a session note before it goes to the payer. Every note Praxis generates is reviewed automatically, you can review a session's note again after editing it, and you can paste any other note — written by hand or copied from another system — into the Review area. Praxis checks it against:
- the Baseline requirements every payer shares;
- the payer's rules in force on the date of service, when you pick a payer. When a payer follows another payer's rules (for example Sunshine Health follows Florida Medicaid (AHCA)), those rules are checked too;
- a documentation-quality checklist a supervising BCBA would use: treatment actually delivered, services within the code's scope, measurable data, complete behavior episodes, numbers that add up, objective wording, and no names of other clients;
- the session data, for a session's note: every number, person, behavior, intervention, prompt level and program in the note must be in what you recorded for the session;
- your own writing style, when you have rules for the service.
Fix with AI then rewrites the note to fix the findings, without adding facts.
Prerequisites
- A Praxis Notes account.
- Optional: rules in My writing style if you want the note checked against them.
Review a session's note
Right after generation
When Praxis finishes writing a note, the review starts on its own. The Note review panel under the note shows each check ticking off, then:
- the verdict and the number of critical issues, warnings and suggestions;
- a card per check: Payer requirements (for example 25/36 met), Session data (details the session data doesn't support), Documentation quality and Writing style. Select a card to see only its findings;
- the top findings. Select one to highlight its passage in the note and see why it matters and how to fix it;
- the rule packs the note was checked against. The payer is the client's primary payer on the date of the session.
After you edit the note
Select Review above the note (or Review again in the panel). The review checks the text in the editor, including edits you haven't saved. When you edit a note after its review, the panel tells you, so you never rely on a review of an older version.
The panel also shows how the verdict moved since the last review, for example Minor fixes → Ready to submit. A review of an edited note starts from the last one: what it found in text you didn't touch stays as it was, unless a detail you added to the session changes it. Problems it notices for the first time in text you didn't change are listed last, under Also noticed in text you didn't change. They still count toward the verdict, but they never make the comparison with the last review look worse.
Select Open full review to see the whole review, with the requirements and writing-style checklists, in the Review area.
The session data check
The session data check compares the note with what you recorded for the session. It flags:
- numbers the session doesn't support: trial counts, percentages, durations, times, dates (critical);
- people described as present who aren't in the session's participants (critical);
- behaviors, interventions, prompt levels, programs and quoted speech the session doesn't record (warning);
- required statements a payer rule asks for, such as "the protocols were implemented as written", when the session doesn't record them (warning). Confirm them, or record protocol adherence on the session form; they're never removed.
Connecting narrative, such as transitions and the setting, isn't flagged. When two checks flag the same passage, you see one finding that names both sources.
Fix with AI
Select Fix with AI on a review, in the session or in the Review area:
- Choose the findings to fix. All are selected by default.
- Select Fix. It costs 1 credit, the same as generating a note. You're charged only if the rewrite succeeds.
- Compare the rewrite with your note. New or reworded sentences are highlighted in green; Your note shows what was replaced. What changed lists the change made for each finding.
- Select Reject to keep your note as it is, or accept the rewrite:
- in a session, Use this version puts it in the editor (save the note to keep it);
- in the Review area, Accept and review again replaces the note.
Either way, the rewrite is reviewed again right away, and the new review shows how the verdict improved.
The rewrite never invents facts. Anything the note (or the session data) doesn't contain becomes a highlighted [MISSING: …] marker for you to fill in from your records. Your writing-style rules apply to what it rewrites.
Review a pasted note
Step 1: Paste the note
Open Review from the sidebar and paste the note (50 to 20,000 characters).
Step 2: Choose what to check it against
- Payer (optional): pick the client's insurance. Without a payer, the note is checked against the Baseline requirements.
- Service: the CPT code of the session (97153 by default).
- Date of service: payer rules are chosen by this date, so late notes are checked against the rules in force on the day of the session.
- Check my writing style: shown when you have writing-style rules for the service.
Step 3: Review the note
Select Review note. The review usually takes a minute or two. You can leave the page; the review keeps running and will be in Recent reviews.
Step 4: Read the result
- Verdict:
- Ready to submit: no critical issues or warnings.
- Minor fixes: warnings worth fixing, nothing critical.
- Needs revision: critical issues that could lead to a denial.
- Review incomplete: part of the review couldn't run. Review the note again.
- Findings, most severe first. Each one shows where it comes from (the payer, Documentation quality or My writing style), the passage of your note it refers to, why it matters and how to fix it. Select a finding, or a highlighted passage in your note, to see the details.
- Requirements: every Baseline and payer rule checked, with its status: met, partly met, missing, added when the note is signed, or doesn't apply.
- Writing style: each of your rules and whether the note follows it. Style never changes the verdict.
Step 5: Fix and review again
Use Edit note to change the note and review it again. Fixes tell you what to add from your own session records; Praxis never invents times, numbers or events.
FAQ
Q: Who can see my reviews? A: Reviews of pasted notes: only you. Delete one from Recent reviews to remove it and the note it stored. Reviews of a session's note: everyone in your organization who can see the session, like the note itself.
Q: Why is a signature or the client's name marked "Added when the note is signed"? A: Those come from the header and signature block of your record system, not from the narrative, so they are not counted as missing.
Q: Does a review cost credits? A: No. You can run up to 30 reviews per hour; the review after generation doesn't count. Fix with AI costs 1 credit per rewrite.
Q: Why is Fix with AI unavailable in the session? A: The note was edited after its review. Review the current text first, so the fix works from findings about what's in the editor.
Troubleshooting
Problem: "You've run 30 reviews in the last hour"
Solution: wait a little and try again; the limit counts the last 60 minutes.
Problem: "The review couldn't finish"
Solution: select Review again. If it keeps happening, contact support.
Problem: "You're out of credits" when fixing a note
Solution: a fix costs 1 credit. Your credits refill with your plan each month; select See plans for more.
